Join Samantha Reinhardt with Doulas Partner for an informative presentation on Enhanced Care Management (ECM), a no-cost Partnership benefit designed to help eligible members navigate healthcare, social services, and community resources. We’ll explain who may qualify, the populations Doulas Partner serves, and how ECM can provide hands-on support with needs such as housing, food access, healthcare coordination, transportation, utilities, and other barriers affecting health and stability. Attendees will also learn how to identify potential ECM members and connect them to Doulas Partner for support.
Join either on Zoom or in person at First 5 Shasta, 393 Park Marina Circle. Lunch provided for in-person attendees!
